Remember Chronology

Employers expect to see job history listed in reverse chronological order, so I think I would list them each, in the correct order, with the other job in between. But for the first time you held the duplicate job (which will appear second in your reverse listing, I would include only the dates/title, etc. Under responsibilities/achievements, I'd just say "see above [give dates]; same exact position".

Another option is to explain it in the cover letter.
